For a $30 copayment, the plan includes one cleaning and one exam per year with an in-network provider. Need more dental care? You can get additional coverage by adding a dental rider to our plans for $35 per month. Refer to the plan specific Summary of Benefits for more details. Hearing Aids
Network Health offers a hearing aid discount program with Simpli Hearing, LLC. Members can find quality brand name hearing aids discounted to $1,220-$1,985. The discount includes a one-year warranty, three office visits, one pack of batteries and one year of loss and damage insurance. You can contact Simpli Hearing at 888-374-6754.

An annual eye exam helps to ensure your health and well-being by spotting early warning signs for a variety of health conditions like diabetes and blood pressure.
